  • Their professional qualification and ICE membership
  • Their employment
  • Their professional relationship with the candidate
  • How the candidate has achieved the ICE member attributes
  • Declaration that you are aware of your candidate's criminal conviction(s) (if applicable)

The form also includes an information on the role and responsibilities of the sponsor.
