  • Their professional qualification and ICE membership
  • Their employment
  • Their professional relationship with the candidate
  • To provide a statement that the candidate possesses the abilities and characteristics for they grade they are applying for
  • Declaration that they are aware of the candidate's criminal conviction(s) (if applicable)

The form also includes guidance on the role and requirements of the sponsor.
