Lessons learned from the purpose-built multi service vehicle (MSV) fire at HS2 Chiltern Tunnels

Overview

A fire occurred during the construction of the down line Chiltern Tunnel on the 10th of May 2022. At the start of a night shift, a purpose-built MSV for personnel was carrying tunnel boring machine (TBM) shift workers and was being driven into the tunnel when a fire occurred in the engine compartment. The MSV was approximately 2.6 kilometers in from the entrance when it was stopped.

Since the incident, a full review of emergency response and ongoing situation has been conducted with emergency services and other agencies at strategic, tactical and operational levels to ensure all lessons learned have been captured.

This presentation will focus on sharing the lessons learned from the MSV fire with the wider industry.

Shannon O’Keeffe CEng MICE

Shannon O’Keeffe is the TBM manager for both Up-Track and Down-Track Tunnels. Shannon has been on the C1 Contract since mid-2021, managing the TBM operations and tunnel back-end works. Prior to joining Bouygues Construction UK, Shannon spent 9 years working for Costain on London Power Tunnels 1, Hinkley Point C Marine Works, HS2 Enabling Works South and finally Thames Tideway East. Prior to relocating to the UK in 2012, Shannon spent over 6 years working for various Tier 1 Contractors in his native Australia. 

Shannon is a graduate of Monash University in Melbourne, a chartered engineer and member of both the ICE and Engineers Australia.

Neil Hancox

A chartered member of IOSH, Neil Hancox has been the hHealth safety & wellbeing director at Align JV since 2018.  His 20+ years of experience of managing health and safety within tunnelling and water sectors has been spent working for major civil engineering contractors and JVs; including, BMB JV (Tideway West), SMB JV (Thames Water AMP6), eight2O alliance, Gifford Try and Morgan Sindall. 

Additionally, Neil serves as the chair of TunnelSkills (the National Specialist Training Group for the Tunnelling Industry group),  is an active members of the Transforming Tunnelling Safety (TTS) group, and a regular contributor to the annual BTS Health & Safety course.
