Overview
David Thomas will share some thoughts on the consequences of failure resulting from several serious incidents that he has had to investigate.
We tend to learn when things go wrong.
As a Principal Inspector with the Health and Safety Executive, David Thomas investigated a wide range of failures in the Construction sector.
A depressing number of investigations were into incidents that had resulted in fatalities.
David will describe several serious incidents that he has had to investigate.
He will share his thoughts reasons for some of the failures investigated. He will describe the consequences of failure and, most importantly, the lessons learned.
